"TLB Refill Exception" on Fuel 600Mhz

I'm seeing this error at cold boot, and when attempting to install an OS.

I've swapped the RAM with known-good RAM.

I've also run the following

(at DEX)> go cac
>clearalllogs
>initalllogs
>flush
>reset

No joy, and symptoms persist.

I have no idea why SGI would have the system throw a scary TLB error instead of saying “hey dummy, fix your console mode.”
