Damage Octane Front Panel

Couple of days ago, I got a second unit with a MXE board . I was Excited until the unit stop working and smoke came out during the second power on. The Octane is dead...after disassembly found out that the MXE Shorted on the socket in the front panel.... The question if someone have this happened in the pass or it was the PSU sending a over voltage...for me PSU must have a system to prevent that....so it was the card....any input or comments....I want to be sure before using that PSU again! it is the good one 733w.

I would suspect more that something was shorted out on the card or at the connector than the PSU was over voltage, though not impossible
